Clinical Referral Criteria

To optimise patient outcomes and ensure appropriate resource allocation, patients referred to Hospice Isle of Man must meet the following criteria:

  • Disease Progression: The patient must have an active and progressive disease where the primary focus of care is the palliation of complex symptoms and the optimisation of quality of life.

  • Service Alignment: The clinical, psychological, or social needs of the patient must align with the specialist services provided by Hospice Isle of Man, establishing us as the most appropriate agency to deliver this care.

Clinical Review Note: If there are ambiguities or doubts regarding a patient’s eligibility for Hospice services, the referral will be formally assessed by our Lead Clinician before services are committed.

Authorised Referrers

Hospice Isle of Man works in close partnership with a diverse network of clinical professionals and multidisciplinary teams across the Island's healthcare sector. To ensure a completely seamless, dignified, and well-coordinated transition into our specialist palliative care services, we rely closely on the vital insights of our community and hospital colleagues. To streamline this intake process, we accept formal referrals directly from the following authorised healthcare practitioners:

  • General Practitioners (GPs)

  • Hospital Consultants (Note: Consultants may refer directly to the palliative care Clinical Nurse Specialist team)

  • Clinical Nurse Specialists (CNS) (Including those operating within the Older Person’s Assessment Service / OPAS)

  • Long Term Conditions Coordinators

  • Senior Allied Health Professionals (AHPs)

The Referral & Intake Process

We have designed our intake process to be as seamless as possible, minimising administrative delays for both you and your patient.

Submit our referral form

All professional referrals must be initiated via our electronic referral form.

Please note: All referrals specifically for lymphoedema management must be completed using our dedicated cancer-related lymphoedema referral form.

Key Worker Allocation

Upon formal acceptance of the referral, a dedicated Hospice Key Worker will be identified and assigned to the patient to coordinate their care pathway.

Direct Access to Wraparound Services

Once a patient is active within the Hospice system, any of our multidisciplinary services can be mobilised for them without further recourse to the original referring source.
